Please read this document carefully. This waiver must be completed for every participant before taking part in any Gude Skillz Lab training, camp, clinic, or program. If the participant is under 18, a parent or guardian must complete it.
Gude Skillz Lab Participant Waiver & Release of Liability
By participating in any Gude Skillz Lab LLC activity, you acknowledge and agree to the terms below. Tap each section to read the full text.
I understand that participation in basketball training, camps, clinics, team practices, strength and conditioning activities, Noah Shooting Sessions, Dr. Dish Shooting Sessions, and other athletic activities offered by Gude Skillz Lab LLC involves inherent risks, including but not limited to falls, collisions, sprains, strains, fractures, concussions, illness, permanent injury, and other physical injuries. I voluntarily assume all risks associated with participation in any Gude Skillz Lab LLC activity.
In consideration of participation in Gude Skillz Lab LLC programs, I hereby release and hold harmless Gude Skillz Lab LLC, Coach Antwon Gude, coaches, trainers, volunteers, employees, agents, facility owners, and affiliates from any claims, liabilities, demands, damages, costs, expenses, or causes of action arising out of participation in any activity, including those resulting from ordinary negligence.
I certify that the participant is physically able to participate in athletic activities. In the event of an emergency, I authorize Gude Skillz Lab LLC staff to obtain emergency medical treatment for the participant if a parent or guardian cannot be reached immediately. I understand and agree that I am solely responsible for any medical expenses incurred.
I acknowledge that it is my responsibility to consult with a physician regarding any medical condition that may affect participation. I understand that participation should be discontinued immediately if any injury, illness, or medical concern arises.
I grant permission for Gude Skillz Lab LLC to use photographs, videos, and other media containing the participant's image for promotional, marketing, social media, website, educational, and advertising purposes without compensation.
Participants agree to follow all rules and instructions from coaches and staff, demonstrate good sportsmanship and respect for others, use equipment properly and safely, and refrain from disruptive, unsafe, or inappropriate behavior. Gude Skillz Lab LLC reserves the right to remove any participant from a program without refund for conduct that jeopardizes the safety or experience of others.
All training sessions, camps, clinics, memberships, and programs must be paid in advance. Training sessions canceled less than 24 hours before the scheduled start time may be forfeited without refund. Missed appointments, camps, clinics, memberships, and programs are non-refundable unless otherwise approved by Gude Skillz Lab LLC.
I agree to indemnify and hold harmless Gude Skillz Lab LLC, Coach Antwon Gude, staff, and affiliates from any claims, losses, liabilities, damages, or expenses arising from participation in any Gude Skillz Lab LLC activity.
This agreement shall be governed by and interpreted under the laws of the State of Texas.
I have carefully read and understand this Waiver and Release of Liability. I understand that by agreeing to this document, I am giving up certain legal rights and voluntarily agree to all terms contained herein.
